Patient Information
Patient Form
If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please call our office if you have any questions at all.

Help With Claims and Estimates
Our team prepares the necessary dental benefit forms and can submit a pre-treatment estimate so you have a clearer idea of your expected coverage before treatment begins.
Coverage May Vary
Insurance policies differ from plan to plan. Your benefits are determined by your provider, and coverage may be lower than expected depending on your specific policy and fee schedule.
Balance Responsibility
You are responsible for any portion not covered by insurance. Claim processing times and payment timelines can also vary between insurance companies.
